She seems to know where everybody is if she wants to know, but can be surprised someone is there when she doesn't consider it (Hanekawa for instance)
She is shown in Vofan artwork surrounded by TV screens, so she may use some kind of omniscience technology.
She has to do research, as she mentions to Koyomi (Koyomin, baby)
She has to send letters to keep in contact with people.
Koyomi keeps surprising her.

So from what is stated here (please let me know if I've forgotten something) she is somewhat inherently omniscient (has the ability to know what she chooses to know).
Hanekawa is starting to display Gaen-like abilities, as is mentioned by Kaiki. Could this be how Gaen got started with knowing?
Is she enlightened?
Is she part oddity like Koyomi? (The Gaen's are known for creating oddities, so perhaps hers is still with her?)
How does she do her research/look around at people?
Where does she live?
How many pies does she have her fingers in at once? The phones are likely for her informants, so it would make sense if she's knows a good deal of strange people.

She's dealing with favors a lot and she seems to have an entire network built around that. Kaiki alluded to it and it was mentioned that you don't want to be on her bad side. At least it would make working in the industry harder.

She seems to have access to knowledge even if she isn't always aware of it all. It seems like she can instantly look any information up inside her "knowledge stash".

It seems to me that she knows everything simply by having really good insight and having a massive information network. I think the insight is something that is shared with Hanekawa and is not due to kaii. I thought it was funny when Araragi calls up Hanekawa for the cram school address she somehow deduces that he's on his way to Kanbaru's.

Doubtful. She explicitly introduces herself to Hanekawa as "I am a human called Izuko Gaen." and while that would immediately bring suspicion upon you normally. She's probably just pre-emptively affirming to Hanekawa that there is indeed a human out there who knows more than you, is more special than you and doesn't need to be a monster to do it like Hanekawa did.

That's the kind of mutual trust relationship they have.
Araragi tries his hardest to be independent so he's not a burden to Gaen and Gaen will use her infinite wisdom to help him through tough times.

To Gaen, what happened between her and Shinobu is obviously in the past, water under the bridge, a minor kerfuffle and shouldn't negatively impact her friendship with Koyomin in any way.
