Kano states that he sees no relation to big picture events yet can relate to the miniscule pieces that make up the big picture.
In saying this he states that he is isolated in this sense and becomes segregated from the rest of the community but in being able to link to another person, in effect becomes linked to the rest of that society.
Now this is where it gets hazy.
He states that in linking with you he confirms his own existence, and in you linking with him confirms his existence. It works both ways.
Make note of the fact that at the final page you take the perspective of a person sitting opposite to Kano, which in the beginning of the manga was a hallucination.
So this means either
A) Kanou lived and we're part of his zany imagination
or
B) Kanou 'died' and he's part of our imagination much like how Kanou had imprinted the persona of the phantom killer
