However, when Decim begins to explain the game, Fujii claims to recall seeing both Decim and the Quindecim.
Decim becomes vigilant when faced with this customer he's never had before.

The woman however also seemed to acknowledge her past and realize that her kids were the most important to her. Decim even complimented them both implying that they understood what happened in their past and realize what's most important to them to move on but she went to void anyway. Both should be reincarnated right?

I think the real tragedy in the show is how everyone experiences some sort of revelation about their lives, only they're already dead, so they can't actually act on any of those revelations.

What went wrong?
I like that the show has this really strong voice, there's no omniscient narrator but you can really feel the director's touch and what he intends to tell you really easily.
