Anime translators think their viewers are stupid. KDrama subbers have so much more respect for their viewers ability to understand "these people are Korean". I never see them try to Americanize the name order.
I think they were attempting to reduce this confusion by making it standard. In everyday situations a Japanese name will often have Western order in written form even if they're also saying it in Japanese order.
In practice I don't really think this works, for the reason you describe. They should have just done like China and mostly left it in Chinese order for everybody. People end up having to guess based on whether they think one name sounds more like a surname.

Admittedly lots of people aren't great at writing clear dialogue, but a lot of weebs also refuse to acknowledge that interpersonal relations are expressed differently in languages where politeness is communicated more through noun and verb choice than grammar particles.
If you mean literally 'Class Rep-chan' and not the original Japanese, then yes that would be pretty weird. But there's nothing unusual about a female class rep getting a -chan from her peers. That doesn't tell you anything noteworthy about the speaker or their relationship. If they were -kun or -sama or -dono or something silly like that, you'd have to either leave it in and assume your readers know the differences or make sure the word choice reflected the attitude.

Obviously a relationship that gets closer could have characters start using more slang or relaxed language around each other to convey the sama -> san kind of relationship. But a lot of that kind of thing require foresight into how the character is going to change to be ready to accommodate such a thing, because other wise it would be like they are a totally different person whipping out a different manner of speaking to the same person just a few days later.
