I was really surprised, I replayed NV not too long ago and found I couldn't kill deathclaws until I found much better equipment. I don't remember having that much trouble with them on release.
Even the Brush Gun doesn't hit very hard against them.

The problem is you eat through nails and cells pretty damn quick, and on some of the later levels, multiple shamblers spawn (Along with even worse enemies backing them up). I almost always have to resort to regular shotgun while popping out around the corner if I'm going to kill them instead of run.
Both of those you suggested do work wonders though, as Shamblers have a 50% resistances to grenades and rockets.
I think the problem is that you are taught to struggle when bound unless told specifically otherwise. With her your first time around, if you struggle you lose. Counter intuitive.
Which is exactly why you conserve ammo for whenever a shambler might pop the fuck out from nowhere.
As for other enemies, smart positioning and gibbing groups works wonders.
Then again, I'm no expert on Quake. Feel like Duke, Doom, Jedi Outcast, Shadow Warrior and a lot of others are overall better.

Parasite Eve 2 had some A-grade nightmare fuel monsters. Part of their creepiness came from the fact that they were all once human (and you can see human features, like faces), before their transformation into artifical neo-mitochondrion creatures (ANMCs). Later you find that some of them were likely volunteers, since it was sold as a shot at eternal youth.
Here's another design: bonesucklers crawl in packs, searching for prey. When prey is found, one of the pack pumps blood to its head until it explodes and the prey is shredded with razor sharp bits of bone. Fucking exploding babies.

I recently played and finished Quake for the first time, on the highest difficulty, and never found ammo to be a problem, but then I always tried to soften the Shamblers with shells while peeking between their lazor charge.
On a different note, I'm actually convinced you can finish this game without ever taking damage, not even from environmental hazards of height.
I got to level 97 of the pit of 100 Trials with non-buffed partners and no gimmicky shit like danger Mario.
This fucker stunlocked me and reset like two hours of progress.
On my second try I got to Bonetail and ended up losing all of my partners and getting down to 8HP on Mario and Bonetail with no Ultra Jellies or Life Shrooms left.
I did a Piercing Blow on Bonetail, it did exactly 8 damage and I was fucking shaking while running to a save block, that shit was way too close.
You really have to ask yourself a few questions if you let a CORE opponent have enough time and respite to build a krogoth. Even then, just spam gunships and it'll go down like your mom on your pop.
A rare Defias Brotherhood leader that was quite powerful for people. Actually she wasn't that powerful and every seasoned player with decent skills and equipment would mop the floor with her.
Problem was that she appeared in the starting zone near Stormwind City.

In Vanilla WoW, in the level 12-20 Westfall area the primary enemy theme was Gnolls and Defias extremists. One such Defias in particular were the pillagers whom were mages and their fireball spells hit PARTICULARLY hard.
Note that in vanilla wow you didn't have all this huge power so you could face tank 5 enemies at once and chaos dunk them. Paladins and Hunters could get away with fighting more than one but most any other class would get shrek'd in about 3 to 4 fireballs. Lets not forget they tend to spawn in clusters in small confined spaces you HAVE to go through.
I know i know, Bordermemes 2 and all, but seriously. You were more likely to kill yourself while shooting at these bastards with anything explosive.

I found that at least on normal, a very large portion of his damage is cold damage. You can drink a thawing potion to temporarily gain 75 cold resist for 20 or so seconds, which makes the fight much, much easier.
He's still a pain in the ass though.

Walking through the inn was a sure way to wind up with multiple mob aggro since anytime they patrolled through doors or down stairs, they were sure to find you. Sometimes, they even threw fireballs through walls for extra fuck you.
Also, I kind of miss that about classic. When soloing in the world, pulling two mobs was pretty frightening unless you were a tank or had an escape, and multiple ones would chew through you quickly and constantly daze you on escape. By the time Cataclysm hit, I was pulling entire areas and AoEing them down like nothing.

That asshole's final fight was such a pain in the ass since he opened up with an attack that dropped your gear's hp to 1 and you had to start the fight burning a chunk of your fuel healing up and killing him quickly before he used wave demiser again.
And you better have fuel saved for the unskippable fight afterwards where the enemy just flat out wrecks your shit if you are not ready.

Thawing potions won't stop the slow/freeze (Not even can't be frozen items can help because it's an entirely different flag that holy freeze uses), but they'll reduce the damage you take.

I haven't played much Black Mesa, but I suspect it's more that the AI is less exploitable. With a lot of older games, they're only really possible if you just fuck with the limitations of the AI, since fighting them toe to toe is just impossible.
Didn't the original ai have some backwards vision mechanic meaning they could see you automatically when they were in your line of sight?

No, it's not. Their AI is literally fucked up. The config file for them has their delay before realizing you are their and their delay before firing set to 0. Plus some other shit making them track you perfectly. If you come out of cover, every enemy alive will begin firing at you with perfect accurately immediately.
