I'm not into that style of cRPG, but that sounds pretty cool.

All I know about my dream game is that it's some sort of mash-up of Muramasa and Dark Souls. I've yet to work out the details.

An Dragon's Dogma 2: Final Ultimate Max Electric Boogaloo with 20 times the amount of content (pretty much almost everything that's on everyone's DD2 wishlist) that spans over many different lands/cultures and many time periods where you can essentially start off as Arthur (Ghosts & Goblins) and end up as Mega Man (figuratively).

Anyway...
A mix between Star Wars Galaxies and EVE Online, with X-Wing vs TIE Fighter style battles, though you could also take command of ISDs or Mon Calamari cruisers if you have enough influence or whatever in either faction (plays kinda like Homeworld, though with a with Silent Hunter like bridges and comm channels through which you give your orders and so forth. There could also be several guys commandeering a single capital ship or have hired NPCs (with levels and RPG customization and shit) take over the different stations.
On the ground it plays out like Star Wars: BF, where you can bring in your armies and fight out the actual battle.
Through trade and economical influence it will also be possible to gain some sort of control over a planet and thus its ressources etc.

My dream game would be a fantasy mmo that has melee combat exactly like chivalry but with loot and magic elements. If your character dies and isn't revived you have to start a new one, players could make money escorting carts through trade routes or through being bandits. There could be a main plot line as well

If you want a game like that, I highly recommend Dark Messiah: of Might and Magic if you haven't played it.

EVE with castles. Essentially just a medieval European sandbox players can com together and form guilds to build castles and mines or do some farming and defend their holdings from rival guild or even join a mercenary company and fight for the highest bidder.

MMO's biggest failing is that they try to hold onto a narrative. From a development standpoint you get put into the impossible position of keeping everyone happy by constantly supplying new patches and content in a timely manner, and most MMO's are not big enough to support this. This is why they fail so easily. The solution is adopt the sandbox style. Just give players the tools and let them create their own story, just like EVE.
