Get a damage balance mod that makes all weapons ( player and NPC ) do more damage
Look for NPC AI mods that make them take cover etc. instead of just running in straight lines at you

I don't know what it would be called in NV since I only used it in F3, but it made the game a lot more threatening and difficult without turning the enemies into bulletsponges.

I also have one, from the 90's. I vaguely remember that it was some sort of colony management / building sim where you had to fight the some sort of disease / alien life forms (didn't have an actual form, look like some sort of goo). I don't remember if you had actual units apart from some transport vehicles or supply ones and all the defense was done with turrets.
