The west is obsessed with identity politics. The people in media (movies, TV, news, etc) being the most obsessed with it. Over the last 20 years we've gone from the media suggesting people avoid certain phrases or words to the media outright ostracizing anyone who says something they don't agree with. As for the traditional sex roles point- that is almost entirely because of SJW counter culture trying to aggressively shove politics into everything.

This is a common misconception.

I've traveled to Japan for work. The general consensus from people I've met there is that the majority of the country is very uptight and conservative (hence their universal ban of explicit depiction of genitalia in all manners of porn and media).

The example you bring up is otaku culture, which is a very specific subsection of Japan's pop culture. It is consumed and produced by a good number of people, but it still represents a very small portion of Japan's overall population. The reason we're exposed to it is because we're also a very niche group of people who have an active interest in pop culture in Japan. Which leads to the misconception that Japan is very open minded about sex and gender roles.

An actual country that actually isn't prude and afraid of the mainstream depiction of sex is France.
