The distribution doesn't follow a clear path. Its entirely possible that a whole Psych ward gains powers while in the next 3 villages nothing changes.
The Epics can expand their powers or even gain new ones. They have to expand their mastery over their original power or survive situations of Great mental and/or physical stress. (Not sure there)
What world would result from that ?

My take:
In some areas the state remains stable. Enough normal people aid the Police/military to counter the Villains/Freaks. This is quickly formalized and the pro-government Epics are brought formally into the fold. Heroes that fight for the Department of Homeland Security (with juicy advertising deals). Police and military form anti-epic response group to aid the heroes or to deal with small scale Villains themselves.
In some areas villains rule. They are either strong or ruthless enough to keep their fiefdoms and to prevent the state from reconquering them. Often they use the threat of civil casualties. This can be everything, from North Korea tier dictatorship worshipping Not!superman ti a crime syndicate not giving a fuck.
And some areas devolve into complete Anarchy. They are either so destroyed from supers fighting that the state has them on the backburner (we will help you of course...once the ressources are available) or locked into a power struggle between different groups.
Scenario ideas:

Your typical older rich tennis club mom gains REALLY strong mind control powers. Her whole fancy schmancy suburb is threatening to sink into Chaos because in the city several gangs of Epics fight it out. At that point she steps in and restores order. The Tennis Club now meets every week again...
The downside: She is completely bonkers because she can't cope with her world collapsing. She viciously mind controls her neighbours to create a paper thin illusion of her old life. If someone disturbs that he is either mind controlled or if that doesn't Work attacked by mindless swarms of mind controlled drones with suicide attacks (her powers don't Work against other epics).

Yeah, Primarchs are strong and I don't really pretend basic Skitarii are worth shit outside of screening the Important Things, but it's not like we're completely without options for dealing with them. Keep a double screen around a core of Dakkastelans and Dunecrawlers and try to unload enough firepower.
Also, we may well get better options in a little bit with the codex, or at least not have to pay quite so much for the ones we do have. In the meantime, I either try to shoot the rest of the army around the Primarch and tie him down or just focus fire the big bastard, and count it as a personal win if I do manage to kill him. It's not like losing an entire army of Admech would faze a Forge World much, they throw that many troops down to hold up a few dozen Marines for an hour in Eye of Medusa.
