Eldrazi are things that do not obey normal physical laws and have a corrosive effect on reality around them, so fighting anything beyond a spawn can be difficult
Side effects of eldrazi presence includes, but are not limited to, spontaneous gravity reversal, possession, insanity and dissolution of matter

This is why they had annhilator, an eldrazi attack would destroy everything around it by the sheer concentration of eldrazi rather than the actual physical might of the creatures present, which is nonetheless substantial

The best way to picture it is to imagine a world of two dimensional figures living on a piece of paper. That is all they know, all they can experience. A very few are able to go to other pieces of paper, but they're still 2D.. Then a three dimensional creature arrives and places the tips of its fingers on the piece of paper, its hand casting a shadow over it.
That is what it is like to fight an Eldrazi. You are fighting shadows and fingertips, while being smudged out of existence and your world is being ripped apart, and at most you can give it a papercut.
But somehow they managed to make 3D into 2D and murder it because stupid plot decisions

We used the seasons of Neptune to work out the year before actually. It's a little vague but should be in the ballpark of ~2130.

In my system when a player runs out of life force they enter a "spent" state in which they cannot act for several turns and are vulnerable to coup-de-grace abilities. After around 5 rounds if combat has not ended they may choose make an endurance based roll to recover a fraction of their LF and return to the fray but failure means an immediate "Bad End." This doesn't always mean actual physical death but regardless requires a revival ritual that costs both the caster an formerly deceased dearly to recover from. Bad Ends that include destruction of the player's body such as getting digested or falling in lava are irreversible save divine intervention - something the Realm's twisted deities are loathe to do and then only with great cost.

Depends on the players, the setting and the style of story you're telling.

Personally, I prefer everyone reaching the end of the story. That doesn't mean they don't struggle to get there, but personally I always feel like death is a boring consequence for failure. It's the end of the story, rather than leading to having to deal with failure or loss of something.

Everyone is John would suit that better, thought. The voices are the mission control, etc.

For an operatives game I'd probably use GURPS with psionics and lotsa military and espionage supplemets or Shadowrun. Still, have exactly no idea how not to railroad operatives.

So I guess you could say we were railroaded?

We used heavily modified Only War, psionics rules were much simpler than DH psykers.

Pretty much this. House rules might make it more interactive by removing the "you can only gain two points per game from combat" rule, but I haven't tried that. Admittedly, some people like this type of economic engine game.
