A ball can be called wide, which is a point and a free swing if the ball is deemed unhittable, which is basically anything behind the batter, over the batters head, or a ball a few feet 'outside'. A wide doesnt count as one of the six balls and is one free run, it's also a free ball if the batter can get to it.
The bowler doesn't have to aim for the wickets and the batter never has to swing or run either. In test cricket especially the batter is usually a lot more defensive and is just trying to not get out at all costs, getting a few runs here and there hoping the bowler messes up. So they often let tough pitches just go by if theyre not going to hit the wicket, or just block them weakly and don't run.
