How did the artist put the logo on there?

Probably edited the model diffuse map in photoshop, easy job

you just need to make 2x all star overlayers, scale em to right size and change perspective match to shoe, select the black areas with magic wand and remove em. then just flat the image. and the result is this, not perfect but just a example.
