Been browsing 4chan since mid-2009 and I don't know how I was able to stand this god awful web page design for so long.
For a proper comparison, you'd need to turn the native extension on, and make the settings as similar as you can. Most of the basics are now in the native extension, but there are a lot of things missing. No archive integration obviously, no expand all images button, no gallery, no way to paste images from the clipboard to the quick reply, no way to auto-watch threads you start or reply to, no way to add any image search tools other than Google or iqdb, etc. The filter UI in native is more user-friendly, though, although you can't filter on all fields.
It's too late, I have gotten used to it now. I can't go back to the time when you had to manually look for replies to your posts.
I use 4chan as it is. The native extension is a part of that now.

I have to make my own scripts, for the most part. The board list, at the top and bottom is too wide for my monitor, for instance. So, I have to set the spacing to like -5px.
Stuff like that.

It used to take up a lot of bandwidth. Which is why 4chan implemented a JSON API that significantly cut down on it. Now it uses less than navigating natively.
Most of those are native now. The guy that developed the native extension took some of the basic user-friendly additions from the popular extensions and added them in.
