Index because I grab the mouse in such a way that it is closer to the scroll wheel.

In fact, most of the time my index finger actually rests on the scroll wheel.

I use index out of habit but middle is objectively better.

Middle finger. My index finger has to stay on lmb at all times since im constantly clicking in like a mad man, slecting text while i read etc.
