Isometrics increase strength in the postion they are held in. Your neck is one of the few muscles that shouldn't be moving around a ton and doesn't need physical muscle mass. We don't train our biceps like this because biceps need range of motion, and we want them to have mass. Neck is better off being strong and tight without adding muscle mass. I'm talking about the front and back of neck, obviously traps are aesthetic.

18 inches, looks pretty normal on me. Never trained it directly though, I think swinging weights from your neck is a pretty dumb thing to do.
