The long answer is the blunt force from the shot rattled the internals of his head against the other side of his bulletproof skull. Similar to how a hard hit can give a football player a concussion, despite a lack of damage to the helmet or the surface of the player's head.

The longer answer is this same mechanic should've killed Luke when he stopped that car from flattening him and Clare, as the blunt force of that impact, despite Luke's toughness and strength, should have sent his internals vibrating inside him like a god-damn cell-phone. It didn't at that point, breaking its own in-universe rules

The short answer is really shitty writing.

I take it that his toughened muscles and body gave him extra cushion (since it extends into his muscles) whereas the shotgun hit the jaw almost directly so it was channeled straight through the bone.

If you recall, OP, in LC it wasn't the shrapnel that was causing the damage after he was shot, it was his flesh that was burrowing it deeper. His own body is the damaging part there.

Not at all how the physics work, anon. Let's assume for a moment that Luke's skin and muscles could have absorbed the impulse of the impact as depicted, his body would have to exert an equal but opposite force almost instantly onto the car. In order for that to happen, Luke's squishy internals (along with the rest of him) would have to shoulder the relatively tremendous force of a speeding truck - something his skin and muscles have been established can do, but something his insides have been established cannot in Jessica Jones.

Luke would have been able to stop the truck, but he immediately should have collapse and began coughing up blood, implying his lungs still fucking exist.

The problem with Luke, unlike say Superman, is his powers don't extend far beneath the surface and it's not clear how deep you go to where he starts being human.
