If anything, they accept that they're female because they have female bodies, which is basically the opposite of what it actually means to be transgender.

Actually, I think it was implied that Xavin actually identified as male. That he at first thought he could just become female permanently to be with Karolina, but later found out it wasn't actually that simple. Because of this he took every excuse to take his male form, for example to be intimidating. It got him into a bit of an identity crisis. Even Molly, normally naive about these thing, saw through this and confronted Xavin about it. I think deep down all of the main cast knew this, but were afraid to say it out loud, fearing that this might make them guilty of ending the relationship between Karolina and Xavin. In the end Xavin realised that while he really had come to love Karolina, he couldn't be with her forever as he couldn't be himself, so this made the decision to sacrifice himself and take her place easier, as this way Karolina would remember him fondly, and he could save his love.

There are certain brain structures that appear more or less depending on whether you identify as a man vs a woman. Truth is that there's a lot of shit we don't understand about the brain, but we do have the evidence that transitioning does help people who are feeling dysphoria.
