Because it requires shapeshifting or a really really good disguise skill, and those are not common abilities.
Not that it wouldn't be funny to have someone like the Punisher have a loli alter ego, mind you, it's just not very likely.

Cross-dressing would be a great disguise if you could pull it off.

Wait, do you mean the super identity as a disguise or the private one? Either could be interesting but the contexts are pretty different. Super-identity would be something like Captain Marvel turning into a girl when he powers up. Private would be like the Martian Manhunter adopting a female alter ego.
Mattie Franklin (the third Spider-Woman) started out by impersonating Spider-Man.
She was a Byrne character, so that was probably him somehow indulging one of his proclivities.
Probably could do reverse trap R34 with the concept, like have MJ or Felicia somehow mistake her for Peter.

It's not always that hard to disguise yourself as the opposite gender. You just need the right build and facial structure. Obviously the Punisher can't be a loli, but with the right hair and the breasts bound a lot of women could pass themselves as men.
My gut is saying trap. Breasts are flat enough they're more like a dude's pecs if that guy isn't unduly athletic. Or maybe I'm being rused and she's just so flat she can masquerade as a guy by putting her hair in a bun under a hat or something

Have an idea for a story/OC worked out.
Shapeshifting D-list superhero is rejected from Justice League analogue, and decides to prove worth by infiltrating supervillain company (Oscorp stand-in) after sneaking a peak at a loose document in their headquarters of suspicion the head is actually a Supervillain (Cat-themed, alliterative)
After taking the guise of a secretary and being hired for a recently vacated position, he learns that workers can hench overtime and becomes catgirl assistant to said villain. In private, learns that the supervillain is actually the son of the original, filling in for sick father and trying to make amends for father's sins. Not only that, hero discovers JL-analogue has been secretly manipulating world events from shadows, and puts out a death warrant on his hero identity. Thus, the hero must become the mask to expose truth.
Also, the longer the MC uses a form, the closer his default form matches it, so secretary becomes neutral shape. Also, animal traits morphed cause minor mental changes, such as cat-ears and tail which are created so the required "wear" for the uniform isn't required

Villain boss takes cues from Green Goblin and Catman/Kraven the Hunter (The Wily Werecat? Baron Bakeneko? Catscratch? Maybe have pair be "Bobcat and Lynx"...)

Not only in animes but in comics, books, videogames, music, etc. Just in case a new villain appears that likes to do crimes based on pop culture, or the Riddler decides to make riddles based in obscure uran fantasy books, or Two Face decides to make a heist based on game sequels with the number 2 on the title, or the Joker decides to imitate hentai manga.
DC used to have a chick who wore a liefeldian bodysuit to pass as a man when superhero/villaining. I honestly don't remember if she was supposed to be good or bad.
