It's a downward spiral. The death of friend was too much for the mind of the MC, so he's giving up to self destruction. And strange sex in a strange place usually reflects that.
The 4th wall creeppy shit makes it more obvious. It reminds us that this is actually part of us, that every human has this basic, ugly, destructive desires. Because that's our nature.
That's why the mouth and the nose looks so realistic in comparison.
