Two things are the only things that people need to find you or prove you committed a crime.

If he is shitting bright red, it is almost certainly something intestinal. The lower back pain would hint at rectal referred pain. The only other possibility that comes to mind could be something like cholecystitis which could also refer pain in that area and cause some of those symptoms.
