Key logger on victims computer
Phishing
Crack email (usually easier) and reset password

I use a script I wrote a while back to do it. The script has a list of popular passwords in it (over 100,000 in the list, another 1,500 once I add first and last name and generate custom passwords)

The script pastes a password, tries to log in: if failed, try next password.
