My stomach feels like it's about to be ripped out my stomach.
I'm on medicaid. But my mom is a deadbeat and won't answer her phone. Is the there a way I can get my insurance information? I have a ride to the hospital and we are heading over now. The last time this happened the pain lasted for like a week.

Adding to this, if you know your provider but not your number, fill it out and guess at the number. Then tell them you did this, and that you may have made a mistake with the insurance number itself, but that you do have it.
