How do I make the pores on my face less visible? Lately they've been really bad for some reason, and I've also broken out into acne, which hasn't happened in a while.

An alpha hydroxy acid product (like one with glycolic acid) to keep them from filling with gunk, then moisturizer and drinking lots of water to keep your skin plump. Plenty of sleep too.
