In anime and manga, there are only so little ways to beat someone who can read your every thought, know your deepest secrets and weaknesses, and can predict your next move. How would you personally beat a mind reader? Or what are your favourite moments in defeating one.
Hard Mode: The mind reader is immune to lewd thoughts because she is used to seeing millions of them.
Harder Mode: She finds your lewd thoughts/secrets pathetic and will use it to embarrass you. If you do not beat her in a swift way, she will spread this to others across the world.

I follow /x/'s advice and build me a tin hat. If that doesn't work I just allow myself to get possessed by a demon, the second she reads my possessed mind the demon now has a direct link to her mind, then she gets btfo'd.

There's only so much that a person can react to. They might be able to dodge a few hits but if you just flail around and move without much premeditation they will eventually get hit.

No one actually consciously thinks their every movement.
Most of your actions are instincts and muscle memories.
If I think that I want to beat her up, then she could perceive that even without mind-reading powers by the time I started charging at her.
Being a mind-reader means shit in combat.
Now, having precognition, that's another caliber, and far more useful.

What if I am in the US and decide to kill her, fly to Japan, find her and shoot her.

You just fight them. Fights aren't planned out in real life, but are mostly based on reacting to your opponent. At most, the mind reader would only be able to predict moves a fraction of a second before they happen, and when you add reaction time, it would be of no help at all.
If it's a woman, as you've indicated, I'd simply beat her up. She would be too weak/slow to put up a fight against a man, and trying to analyze your opponents moves a split second before they happen would be more of a hindrance than a help.
For the sake of assumption, let's just say she's in a battle with you (of physical or some game, your choice) and can only read one mind clearly at a time.
So far, I think that the only legit ways to beat a mind reader is by teamwork or gang attack, as stated in >>137452678. One person with multiple thoughts isn't going to work because you'll always have a higher focus towards the true thought of action. But having two people (presuming that they can't mind-read each other) will work by their own different actions and team-p against the mind-reader.

Liked that part in Yu Yu Hakusho. Mind reading doesn't matter if the guy doesn't have the physical abilities to keep up with his target (which should logically be the case for people relying on mind reading ).
