For three days straight, without any food and very little rest, Mikoto had been destroying research facilities. By the time Mugino got to Mikoto she had been so worn down that she couldn't actually do anything. And still kicked Mugino's ass with the dolls.

Mugino has a tendency to get tunnel vision and underestimate her opponents, which puts her at a disadvantage with nearly anyone she fights. (Hamazura being the next best example). Lets not forget the more power and stress Mugino puts on herself to use Meltdowner the higher chance she has of an accident, which is exactly what happened against Hamazura.
