Only a few characters are such an extreme case as Patchouli though.
Remu and Marisa are pretty consistently drawn with small tits. Yukari usually has big or at least modest tits. Remi, Flan and the other lolis are consistently drawn flat too. There are of course exceptions and it's not hard to find an image of Remilia with huge tits, but that's only because the touhous get so much art. Patchy is literally 50/50.
