Basically, there are limits to the sheath's power. Its way of mending Shirou's wounds is incredibly roundabout due to his Origin and Attribute, so there's a chance it could actually end up killing him, as it does when Rider kicks him out the window and he gets skewered from within by the swords that are stitching him back together.
