Touma isn't a very proactive guy. Out of all of his current relationships with various people, he himself puts in no real effort to maintain any of them. He doesn't try to seek anyone out, he never calls anyone and asks if they want to hang out, and he never goes out of his way to act friendly towards anyone. If he sees someone, he'll be friendly towards them, but he'll generally only contact someone himself if he needs some help from them.

The exception is when he feels someone's in trouble and wants to save them. That's the only condition in which he'll go out of his way to approach someone. But if they're okay and safe, then he'll never approach them on his own.

Index is a special case. Throughout the first few months of his life, she was basically his anchor, the one thing that kept him afloat after he lost his memories. Even if he doesn't view her as a lover, she's still a very important existence to Kamijou Touma.

In more recent volumes, he also appear to regard Misaka Mikoto as something special too. Again, not as a potential lover, but she's one of the few people who proactively attempt to support him, not just physically, but emotionally. The narrative makes it clear that her continued support is important to Touma, something that actually touches him. It's not enough to make him see her as a potential lover, but it's enough to make him be particular about her amongst his circle of acquaintances. At the very least, she's definitely someone he regards as a "friend".

I consider Kamijou Touma to be a very warped individual. Much like Emiya Shirou, who was posted somewhere in this thread, he is someone who cannot experience joy or happiness from simple things. Yes, he may be at peace or act normally in an every day school life, but he does not derive true happiness from it. Touma's life is full of fukou da. That scene represents him finding a break from fukou da thus his state of being content by being pleased from seeing a girl being sweet enough to cook for him. If you can twist respect and whatnot into not liking, then surely you can twist being pleased into not liking cause state of mind, etc. People can argue that when you get down to it, like or not has more to deal with being close rather than pure satisfaction for the small things. That's why characters like Othinus and Index should be at the top of Touma's "liking" despite all the shit they give him.
